What kinds of questions should be included in a curriculum needs essment survey?

I'm doing a mock-up of a curriculum needs essment survey for a school that has not yet opened. This particular survey is directed at our English curriculum for grades 1-6. I'm at a loss for what questions I should be including that would best help us articulate our English curriculum. The survey is directed at parents, faculty, a the various school boards, in order for us to figure out what is specifically needed in our English program.
